About Department and Purpose of the Position

The (Senior) Assistant Manager / Executive is responsible for the management of assigned program under NUHCS as well as submission of audit data on a timely manner.

 

Duties and Responsibilities

1) Program Coordinator Role

· Responsible for day-to-day operation of Transitional Shared Care Programme (TSCP) for cardiology patients, including the following:

a. Screen patients in cardiology clinics for suitability for TSCP based on entry criteria for each condition group.

b. Approach clinician in-charge of the patient to review patient’s suitability.

c. Recruit patient into TSCP.

d. Liaise with care coordinators in primary care settings (NUP, Primary Care Networks and GPs) for booking of follow-up appointments.

e. Communicate treatment targets of the patient with primary care partners.

f. Coordinate follow-up appointments at cardiology clinics.

g. Provide necessary follow-up with patients who defaulted the appointments.

h. Provide support and health counselling to patients.

 

· Track performance of TSCP

a. Create database to track all patients, who are enrolled in the programme.

b. Record patients’ attendances at both primary care settings and cardiology clinics.

c. Track and document each recruited patient’s progress and ensure compliance of the clinical protocol.

d. Record achievement of treatment targets for all patients.

 

2) Clinical Audit Coordinator Role

· Collect acute myocardial infarction mortality data and ensure timely submission to MOH.

· Prepare reports and timely submission to the Head of Department and Medical Affairs-Clinical Governance Office.

· Collect the diagnostics variables, procedure medication, intra & post procedure events, comorbidity and lab results as well as treatment plan (optimized medical, revascularization procedure).

· Prepare and submit audited data to the respective authority accurately and timely.

· Perform additional Operations and Administrative tasks as assigned by the supervisor.

 

Key Requirements

· Bachelor’s degree from a recognized institution.

· Enrolled Nurse / Registered Nurse / Allied Health Professional / Staff with relevant clinical knowledge will be advantageous.

· 3-5 years’ working experience in healthcare settings.

· Good verbal and written communication skills.

· Strong interpersonal skills with the ability to engage and work with all levels of staff.

· Have a positive and proactive mindset.

· A good team player who can work independently.
