AWESOME (Adolescent Wellness Service - Options Made Easy) is a part of the Child and Maternal Health and Well-being (CAMH) Strategy, which drives for better health outcomes for children and their families and supports our youths to achieve their maximum human development potential. The programme aims to:
- implement a community adolescent health initiative to provide preventive health screening for adolescents and their families;
- leverage existing youth support services provided by youth workers and healthcare professionals; and
- enhance existing resources on adolescent health to increase health literacy and awareness.
Primary Responsibilities
- Relationship management: to build strong relationships with internal and external stakeholders (e.g., patient and/or family/caregiver)
- Screening: use appropriate screening tools to identify patients/clients who could benefit from the programme (e.g., mental health screening and assessment).
- Care Coordination: provide patient/client counselling/coaching, develop appropriate interventions and ensure closed loop referrals and follow up (e.g., referral to community care partners/programmes).
- Care documentation: ensure timely and accurate care documentation.
- Development of care workflows: work with a multi-disciplinary team to develop and continuously improve care workflows, to deliver optimal programme outcomes.
Secondary responsibilities:
- Cross coverage for NUWoC Community care coordination duties.
- Support for NUWoC and NUH related events.
- Any other duties as assigned.
Requirements:
- Degree in Psychology, Nursing, Social Work, Counselling or related Allied Health qualifications preferred/
- 3-5 years experience in Psychology/Counselling and/or working with youth and families in Singapore would be an advantage.
- Possess critical thinking, communication and interpersonal skills to work in a multi-disciplinary team.
- Disposition to work with youth and their families.
- Meticulous and strong attention to detail, ensuring end to end care coordination and follow up.
- Equipped with basic computer skills in MS Word, Excel, and PowerPoint.
- Familiarity with relevant community services will be an advantage.
