Case Management Officer (Renal Centre)

Overview

Disease specific case management for patients with Chronic Kidney Disease (Stages 1-5) and to assist patients and their family on the various treatment options available for End Stage Kidney Disease.

Job responsibilities

• You will be responsible for the following:
• Assess, plan, implement and evaluate care of patients with renal disease
• Conduct teaching and counselling for patients and their families / carers
• Co-ordinate patients’ access and treatment sequence where required
• Assist with overall coordination of renal replacement therapy
• Assist with placement of patients in dialysis facilities as required
• Deliver patient care (inclusive of education and counseling) according to the established standards of care and practice using patient care plan / clinical pathways
• Contribute to multidisciplinary patient management
• Apply understanding of prevention of medico-legal issues in relation to patient care
• Collaborate and co-operate with physicians and other health-care providers in the delivery of patient care and services

Requirement

• Degree in Nursing, Biomedical Science, Life Sciences, Biotechnology or General Science
• Preferably relevant experience in healthcare setting
• Good verbal and written communication skills
• Possess confidence to do counselling with patients and family
• Ability to multi-task and work under tight timelines
• Good analytical and organization skills
