About NUHS Regional Health System Office

The NUHS Regional Health System Office (RHSO) is part of the National University Health System (NUHS), a leading academic health system and one of three public healthcare clusters in Singapore. Leveraging its position within an academic health system and drawing on the system’s research and education capabilities, RHSO strives to build a healthy and engaged population in the west, empowered to live and age well in their homes and within the community.

 

Through a multi-pronged approach, RHSO brings together NUHS institutions, as well as community health and social care partners to develop and implement initiatives to improve health and anchor care in the community. RHSO also facilitates the integration and transition of care between the hospital and community, as well as the development of new capabilities to deliver care in the most appropriate setting.  

 

Do you have a passion to serve the community during your semester break?

 

We are looking for individuals who are keen in the following area:

 

1. Care Coordination

You will support the department in the coordination of care for residents in the community.

 

You can play a part in

i) helping in the enrolment of patients

ii) engaging with the residents

iii) participating in the health screening and wellness to the patients through care planning and coordination of care in the community

iv) providing administrative support such as organising events, data consolidation and etc.

 

2. Operations

The Operations is made up of many divisions from Resident Outreach & Engagement, My Health Map, Community Care Operations and Community Development & Partnerships with the aims to improve and anchor care in the community.

 

The Resident Outreach & Engagement team aims to empower the residents to take care of their health with the aim to develop long relationship between the community and the Healthcare providers.

 

My Health Map (MHM) focuses on informing and promoting health screenings to residents to prevent chronic diseases. Residents can sign up MHM to enjoy a slew of health advices to gain knowledge in self-care.

 

The Community Care Operations team aims to increase the accessibility of preventive health programs for residents and improve the integration of social health services.

 

The Community Development & Partnerships team collaborate with community care partners to deliver appropriate care in the community and plan healthy precincts to support residents in the adoption and sustaining of healthy lifestyles.

 

You can play a part in

i) engaging with residents in taking charge of their health

ii) supporting in community events and health programmes/screenings

iii) providing event support such as promoting in NUHS initiatives (e.g. MHM, CHP)

iv) helping in implementing the Healthy Precincts

v) advocating good self-care management

 

3. CareHub

The CareHub teams is responsible for many healthcare innovative projects such as NUHS@Home, Telehealth, Chronic Disease Management Programme (CHAMP) and Care & Call Centre.

 

NUHS@Home provides hospital care which is similar to the treatment received at the hospital ward for patients but in the comfort of their own home.

 

Telehealth and CHAMP uses IT, AI and behavioural science capabilities to support chronic disease management (CDM) and patient compliance at the population level.

 

Care & Call Centre is one of the virtual enablers to support NUHS Population Health efforts and empower residents so better manage their own health.
