Executive / Senior Executive, Intermediate and Long Term Care, RHSO

Reporting to Manager, ILTC, the Position Holder will be responsible for engagement of Intermediate & Long Term Care (ILTC) providers and management of collaborations between NUHS and ILTC providers in the community to enhance care integration and delivery of care in the community.

Job Responsibilities

You will be responsible for the following:

• Regular engagement with key account community partners to improve care integration and delivery of care in the community
• Management of feedback and issues with/from community care partners
• Management of collaborations/initiatives/projects to improve care integrations between NUHS and ILTC providers and/or augment community care
• Keeping abreast of developments within ILTC and community care organisations
• Serving as workgroup/meeting secretariat
• Supporting implementation of NUHS Population Health strategies and regional health plan
• Provide adminstrative support to department activities and programmes

Requirements

• Bachelor Degree, preferably in Life Sciences, Business Administration or related qualification
• Fresh graduates are welcome to apply, prior experience in community care or partnerships will be an advantage
• Experience in project management and working with multiple stakeholders
• Possess confidence to engage internal and external stakeholders at all levels
• Ability to multi-task and work under tight timelines
• Good analytical and organization skills
• Excellent written and verbal communication, problem solving, presentation, negotiation and interpersonal skills
