The incumbent will assist the Registered Nurses/Enrolled Nurses to provide basic care for patients in inpatient areas.
Job Responsibilities
- General Hygiene and Patient Care Activities
- Attend to patients’ call bells
- Assisting patients with elimination needs, includes application of urosheath
- Apply anti-embolism stockings
- Prepare patient for physical examination and/or other procedures
- Change bed linen and patient’s clothing
- Perform assisted baths
- Perform perineal skin care
- Perform last offices
- Assists with lifting, transferring, positioning and transporting of patients
- Nutritional Needs
- Document intake and output
- Order patients' meals
- Serve patients' meals and perform oral feeding
- Remove nasogastric tube (NGT)
- Assist to feed patient via NGT/ PEG
- Perform continuous tube feeding
- Basic handling of continuous feeding pump
- Admission Procedure
- Prepare for patient admission
- Measure patients' height, weight and vital signs for baseline
- Orientate patient on ward environment and visitor policy
- Perform initial assessment on patients
- Inter-Departmental Transfer
- Set up, handle and replace portable oxygen tanks
- Clinical Test & Vital Sign
- Perform vital signs monitoring and record measurement
- Perform 12-lead electrocardiogram
- Perform capillary blood glucose monitoring and record measurement
- Collect urine, stool, sputum specimens and dispatch
- Collect MRSA swabs (nasal, axilla, groin) and dispatch
- Perform safe measure of urine from urinary catheter
- Drug Administration/Therapy
- Administer oxygen therapy
- Nursing procedures
- Assist with application of physical restrainers
- Remove peripheral venous cannula
- Re-position bed bound patients 2 hourly
- Assist with oral suctioning
- Assist with complex wound dressing and bandaging
- Assist with IV cannulation and venepuncture.
- Assist with initiation of emergency procedures
- Perform bladder scan
- Assist doctor / nurse in minor surgical procedure
- Patient fronting tasks
- Manage visitor access to patients in the ward
- Provide teaching to patients and relatives on infection control practices
- Follow up on planned discharges, obtain and follow up on appointments for all discharged patients
- Manage calls and coordinate between wards and various dept/ NOK
- Bed Management
- Perform headcount audit of patients in the ward
- Request beds for transfer out of patients
- Update patient movements out of the ward, example transfers and discharges
- Medical Records
- Request for medical records and X-rays electronically, if required
- File casenotes and compile casenotes in the ward for transport to HIMS/MRO
- Inventory management
- Perform inventory count and asset tracking
- Top-up supplies
- Perform fire safety checks on a monthly basis
- Service
- Engage patient and family in ward orientation and visitor policy
- Process review
- Participate in QI projects
- Clerical support tasks
- Handle incoming and outgoing mails, and coordinate with porters to dispatch and receive items or supplies
- Conduct discharge of simple cases
- Collection of patients' property
- Perform daily ward rounds to update:
- Patient's name updated in headboard
- Attending Doctor/Discipline updated in system
- Choice of class and acuity (e.g. HD/ICU/GW)
- Portering
- Transport patients for urgent procedures with nurse
- Porter urgent cases (patients) on wheelchair
- Collect and dispatch of items in urgent situation
- Housekeeping
- Maintain a safe work environment and patient area
- Maintain cleanliness & tidiness of work and patient environment
- Perform proper waste disposal including handling of spillage
Qualification Requirement
- Minimum NITEC / Higher NITEC and equivalent
