Medical Social Worker, Psychological Medicine, REACH (Contract)
Job Purpose
- To improve mental health of children & adolescents in schools by providing early
intervention and assessment to students - To equip school counselors with skills to manage students with mental health
conditions at school, and - To develop a mental health network for children and adolescents in the community
involving:
a. General practitioners (Family doctors and community pediatricians)
b. School Counselors
c. Social Service Agencies (SSAs) and Family Service Centers - To provide clinical social work intervention in the areas of counselling and support, including parenting work and family therapy sessions for REACH patients
- To provide psycho-emotional, socio-economic input to the multi-disciplinary health
team in order to achieve holistic patient care
Duties and Responsibilites
Primary Responsibilities and Duties
Clinical Role
- Conduct bio-psychosocial assessments to determine selected students’ current mental state.
- Provide psychological support and intervention to the referred students and caregivers.
- Monitor and evaluate referred students’ progress and revise treatment/care plan as needed.
- Financial Assessment and Assistance to patient who require financial aid for treatment.
Case Management Role
- Provide support and training to school Counselors through telephone advice / consult and guidance in implementation of therapeutic interventions.
- Formulate and develop care management plans for referred students in collaboration with school counselors and other community partners.
- Support clinicians in right-siting suitable students to clinic for follow up.
- Coordinate all aspects of transfer of care, from referral to the first clinical visit and monitor the right-sited student’s attendance.
- Collaborate with clinicians to evaluate the student’s adherence to the treatment.
Liaison Role
- Initiate appropriate referral and liaison with other healthcare professional and agencies.
- Establish a network of SSA resources and information for dissemination to the referred student/ caregiver and school counselors.
Secondary Responsibilities and Duties
Quality Management
- Assists in formulation and revision of guidelines, standards and procedures
- Participates quality improvement projects
Requirements
- Degree in Social Work / Degree in Arts, major in Social Work.
- 2 years of relevant work experience preferred.
- Good communication and interpersonal skills.
- Competent in IT skills.
