Job Description:

NUHS@Home is a new and innovative hospital-at-home programme that is transforming healthcare delivery to patients anytime, anywhere. Our multi-disciplinary teams provide hospital-level acute care to patients who would otherwise require inpatient hospitalisation within the comfort of their own homes. We do this with a combination of technology enablers such as remote patient monitoring and video consultation, home visits and point of care investigations.
 

We are seeking a highly motivated and compassionate Associate Consultant/Consultant to join our team. As a Consultant in Advanced Internal Medicine, you will be responsible for providing leadership and clinical oversight to the NUHS@Home team, while working closely with other medical and allied health professionals to provide the highest quality of care to our patients and transform the care delivery landscape. We are looking for doctors who are agile, innovative, and proactive problem-solvers, with the passion and conviction to transform the way we deliver healthcare, bringing professional medical services out of the hospital into the patient’s home.
 

NUHS@Home is a service under the Division of Advanced Internal Medicine, Department of Medicine, National University Hospital.
 

Responsibilities:

    • Provide clinical leadership and oversight to the NUHS@Home team, including junior doctors, nurses, pharmacists and allied health professionals to provide the highest quality of care to our patients.
    • Conduct patient assessments, develop and implement care plans, and provide ongoing medical management of acute illnesses.
    • Participate in daily virtual team rounds with the interdisciplinary team to review patient progress and update care plans.
    • Communicate effectively and empathetically with patients and their families to ensure they understand their care plan and are comfortable with the care being provided.
    • Utilize technology enablers such as remote patient monitoring and video consultation to enhance patient care and safety.
    • Conduct home visits and point of care investigations as required.
    • Ensure compliance with NUHS@Home policies and procedures, and maintain accurate and complete medical records.
    • Participate in research and quality improvement initiatives to continuously improve the NUHS@Home programme.

 

Qualifications:

    • Basic medical and postgraduate qualifications from the accredited medical universities or medical schools which are registrable with the Singapore Medical Council;
    • Must be successfully registered with the Singapore Medical Council and accredited by the Specialist Accreditation Board, Ministry of Health;
    • Completion of advanced specialty training in Internal Medicine, Emergency Medicine or Family Medicine 
    • Willingness to embrace new technologies and innovative ways of delivering care.
