Job Description:

NUHS@Home is a new and innovative hospital-at-home programme that is transforming healthcare delivery to patients anytime, anywhere. Our multi-disciplinary teams provide hospital-level acute care to patients who would otherwise require inpatient hospitalisation within the comfort of their own homes. We do this with a combination of technology enablers such as remote patient monitoring and video consultation, home visits and point of care investigations.

We are seeking a highly motivated and compassionate Resident Physician to join our team. As a Resident Physician, you will work under the supervision of a Medicine Consultant and be responsible for providing hospital-level acute care to patients in their homes. We are looking for doctors who are agile, innovative, and proactive problem-solvers, with the passion and conviction to join us as we transform the way we deliver healthcare, bringing professional medical services out of the hospital into the patient’s home.

NUHS@Home is a service under the Division of Advanced Internal Medicine, Department of Medicine, National University Hospital.
 

Responsibilities:

  • Conducting patient assessments and history-taking, developing and implementing care plans, and providing ongoing medical management of acute illnesses
  • Participating in daily virtual team rounds with the interdisciplinary team to review patient progress and update care plans
  • Communicating effectively and empathetically with patients and their families to ensure they understand their care plan and are comfortable with the care being provided
  • Utilising technology enablers such as remote patient monitoring and video consultation to enhance patient care and safety
  • Conducting home visits and point of care investigations as required
  • Ensuring compliance with NUHS@Home policies and procedures, and maintaining accurate and complete medical records
     

Qualifications:

  • MBBS or equivalent medical degree recognized by the Singapore Medical Council
  • Eligibility for registration with the Singapore Medical Council
  • At least 2 years of postgraduate clinical experience in an acute care setting
  • Excellent communication skills and ability to work effectively as part of a multidisciplinary team
  • Compassionate and empathetic towards patients and their families
  • Willingness to embrace new technologies and innovative ways of delivering care
  • Ability to work independently and make quick decisions in a fast-paced environment
