Overview

The Job Holder will support the Clinical Services Planning & Integration (CSPI) Division including specialist and primary/community care co-leads, and work with Group Care Plan Operations and other internal and external stakeholders to facilitate the development of cross-sectoral, end-to-end care plans with emphasis on prevention for target conditions, for NUHS Population Health Strategy 6 (pathways for conditions with significant disease burden).

 

Job Responsibilities

(1) Project management oversight for end-to-end care plans
Provide secretariat and administrative support to CSPI management, specialist and community care co-leads to facilitate the development of end-to-end care plans for target conditions, including:
o Supporting mapping of existing services, and identification of gaps
o Keeping abreast of NUHS and community services, checking for alignment to national and NUHS plans and guidance
o Working with relevant community partners needed for care plans
o Supporting development and implementation of care plans with focus on linkages with community and primary care, proposals/workplans and reports

 

(2) Support implementation of NUHS Population Health strategies and regional health plan
• Support CSPI management in the development of long and short term strategies and plans for respective areas of work, ensuring alignment with overall RHSO and NUHS’ strategies and goals
• Work with CSPI management to develop and translate strategy into new work plans
• Keep abreast of NUHS Population Health developments and identify opportunities/gaps/support where appropriate
• Actively sync and identify potential areas of collaboration with NUHS and community to support population health

 

(3) Leadership and team management
• Manage a team of 3 staff, as well as talent development and succession planning
• Support CSPI management to track and ensure compliance to corporate and clinical governance
• Work with CSPI management to set priorities and articulate strategies and goals to all direct and indirect reports
• Work with CSPI management to develop, engage and motivate an effective CSPI team to deliver KPIs/milestones
 

(4) Any other task as assigned

 

Requirements
• Good degree in any discipline, preferably in Life Sciences, Business Administration etc.

• 8 to 10 years of relevant working experience, with 3 - 5 years in a leadership position
• Experienced in project management
• Experience in community care or partnerships will be an advantage
• Possess confidence to resonate with senior management leadership – internal and external stakeholders
• Ability to multi-task and work under tight timelines
• Good analytical and organization skills
• Good command of English, with experience in writing proposals and papers
• Excellent communication, problem solving, presentation, negotiation and interpersonal skill
