The NUHS@Home nurse provides nursing care to patients who require acute medical treatment but who are able to be cared for in their homes rather than in a hospital. The nurse works in collaboration with physicians and other healthcare professionals to develop and implement care plans, monitor patient progress, and educate patients and their families on self-care techniques.

The NUHS@Home nurse is competent in performing care assessment and planning, and in the management of patients with identified care needs. S/He provides clinical supervision, precepts the junior nurses and contributes to NUHS@Home nursing training.

 

Job Responsibilities

 

Primary Responsibilities and Duties (80%)

  • Conduct patient assessments and develop individualized care plans
  • Administer medications, treatments and procedures as ordered by the physician
  • Monitor and evaluate patient progress and adjust care plans as needed
  • Communicate with physicians, other healthcare professionals, patients and families regarding patient care
  • Educate patients and families on self-care techniques and disease management
  • Ensure patient safety and infection control measures are maintained
  • Document patient care and progress in electronic medical records
  • Participate in quality improvement initiatives
  • Adhere to ethical and legal standards of nursing practice
  • Utilise technology in care delivery

 

Secondary Responsibilities and Duties (20%)

  • Participates in research, clinical or service quality activities
  • Demonstrates awareness of current nursing and health care practices
  • Assumes additional responsibilities as assigned by Nursing Officers, Senior Nursing Officers, Assistant/Deputy Director (Nursing) and Chief Nurse

 

Job Specification/Requirements

  • Possess Diploma / Degree in Nursing and must be registered with Singapore Nursing Board
  • Registered Nurse with minimum of 3-5 years of post-registration clinical experience
  • Ability to work independently and as a team
  • Strong organisational, coordination, interpersonal and communication skills
  • Ability to work a flexible schedule, including evenings, weekends and public holidays
