ACP Care Coordinator (Contract till 31 March 2027)
Overview of Position
-
Facilitate Advance Care Planning (ACP) for targeted groups of patients in JurongHealth Campus (JHC) and clients of JHC’s community partners.

Advocate ACP to patients, care-givers, healthcare professionals and the public.
-
Support the development, implementation, optimization and integration of ACP across JHC and NUHS, aligned to national ACP guidelines.
Responsibilities

Contribute to patient care by facilitating ACP discussions and completing advance care plans that meet patients’ needs
-
Ensure patients and their families/care providers and healthcare stakeholders have access to patients’ ACP documentation via soft/hard copies of the ACP Form.

Seamless and timely access to the documentation on the patients’ ACP Discussion Worksheet to all care providers.

Facilitate appropriate and timely review and update of patients’ Advance Care Plans with patients/their Nominated Healthcare Spokespersons

Provide relevant feedback loop to address service gaps and service delivery issues.

Support the national KPIs to honour patients’ care preferences

Support seniors in reviewing the ACP implementation processes/IT systems to continually improve the feedback loop to address service gaps and service delivery issues
-
Support the gathering and submission of ACP data/statistical outcomes on a timely basis and adherence to the processes

Present statistical outcomes at appropriate platforms within JHC when called upon

Support logistical and other requirements for all ACP events including meetings, training and networking initiatives to contribute to the successful completion of the events

Increase awareness of ACP in the community and contribute to higher acceptance and completion of individual care planning by the community

Contribute towards raising the competence of ACP Advocates/Facilitators within JHC/JHC community partner organisations

Contribute towards the implementation of ACP education on national platforms e.g. contribute as a team member to write articles for ACP newsletters.

Contribute knowledge/skills for research/quality improvement projects

Participate in collaborations on projects with clinicians and other stakeholders
-
Apply knowledge gained to improve the JHC ACP programme
-
Perform any other duties as assigned by the Reporting Officer
Requirements
-
Bachelor’s degree from a recognized tertiary institute in Nursing, Psychology, Social Work, Counselling or other Allied Health disciplines relevant to the job role.

A keen learner, critical thinker, self-motivated, who works well as a member of a multi-disciplinary team
-
Good interpersonal, communication and multi-tasking skills.
-
Able to communicate proficiently in local languages/dialects.
-
Proficient in Microsoft applications, including MS Word, Excel, and PowerPoint.
-
Experience in Psychology/Counselling in Singapore or related fields, familiarity with relevant local healthcare/community services and/or experience with research/quality improvement projects will be an added advantage.
