Assistant Manager, Referral Management
Overview
The Assistant Manager assist to ensure the smooth and efficient operation of the Referral Management Department in JCH, to deliver rapid, appropriate, and safe transfer of patients from referral sources to JCH, in a patient-centric manner.
Responsibilities

Assist to ensure smooth operations of the department.

Exercise judgement and people-manage to resolve conflicting needs for patients/ staff.

Able to coach and supervise Executive/ Sr Executive and Sr Executive Assistant to always ensure smooth operations.

Assist to review and update department's policies and procedures, data inventory map, risk management, department's organization chart and staff JD.

Participate in Licensing and Accreditation.

Assist in SQ enquiry pertaining to referral management.

Support JCH in related projects and initiatives.

Participate and perform in other duties as assigned.

Possess good knowledge of JCH admission policies.

Carry out assessment of referrals from (NGEMR, AIC-IRMS or paper referral from other sources) timely with the support of JCH's clinical team.

Ensure proper documentation of all transactions into the IT system or / and case notes, accurately and timely.

Ensure safe and positive patient's journey when transiting patients into JCH.

Protect patients' data and maintain strict confidentiality.

Assist and attend to email, phone call and walk in enquiries.

Assist to mentor and precept new staff.

Function as Point of Contact for Referral Source.

Foster partnership and interact effectively with referral source.

Optimize JCH beds utilization and BOR.

Responsible for department's KPIs.

Maintain excellent clinical documentation, as well as a database of referrals, outcomes, and logs of activity, with the aim to audit, and optimize referral management.

Attend continuing education, whenever relevant.
Requirements
-
Degree in Nursing or Health Science related.

Post-graduate training in relevant specialty is preferred.

Proficient in English and at least 1 other local language.

Proficient in Microsoft Office applications.

At least 5 years of relevant clinical experience preferably with at least 2 years of relevant Community Hospital experience.
