(1) Overview and purpose of the position*

Care Coordinator

for the Geriatric Hematology Oncology Longitudinal End to eNd (GOLDEN) Program

 

Patient population

Older Adult cancer patients aged 65 and above

 

Roles and responsibilities

I.            Administration of a comprehensive geriatric assessment and screening tools for frailty

II.           Facilitate the provision of end –to- end care for cancer patients in the geriatric population

III.         Assist the geriatric oncology patients in navigation of the healthcare system

IV.          Facilitate weekly MDR discussions for geriatric oncology patients

 

Key Competencies needed

I.            Experience in managing older adults with cancer

II.           Familiarity with frailty screening tools (e.g., G8, CFS)

III.         Conducting or coordinating Comprehensive Geriatric Assessment

IV.          Treatment toxicity risk stratification

V.           Multidisciplinary team coordination

VI.         Advance care planning discussions

VII.        Navigation of community services

1)      triage for a more comprehensive geriatric assessment

·       using the Geriatric 8 and Clinical Frailty Scale (CFS) screening tool

 

2)      To conduct comprehensive geriatric assessment (CGA) by using the following tools for the respective domains

  • Demographic and social support status
  • Functional status : Katz, Lawton scale, Fall questionnaire and Time up and go test
  • Sensory functional status (vision and hearing)
  • Cognitive function ; Mini COG Test, Clock drawing, Mini Mental State Examination
  • Emotional status : Geriatric depression scale
  • Nutritional status : Mini nutritional assessment
  • Performance status : Karnofsky performance status scale
  • Caregiver burden status : Zarit Burden Scale to assess the burden to the family members
  • Polypharmacy status

 

1)      To provide a detailed evaluation on medical, psychosocial and functional issues and to identify areas of vulnerability and frailty based on the results generated from the comprehensive geriatric assessment.

 

2)      Maintain accurate records of patient’s medical history, treatment plan and progress

 

3)      To participate, discuss and collaborate with other allied health providers in a multidisciplinary discussion held weekly to determine interventions required based on the comprehensive geriatric assessment

 

4)      To collaborate closely with the multidisciplinary team to optimize care needs and ensure continuity of care for the patients.

5)      Identify and provide emotional and psychological support for patients through referral system to specified allied health providers e.g:

  • Allied health service teams: Occupational Therapist, Physiotherapist, Medical Social Worker, and Dietician
  • Community resources

 

6)      Assessing on-going needs and educational deficits of patients and their caregivers.

 

 

2. ADMIN DUTIES

Indirect patient care

1)      Work in collaboration with the geriatric oncology team to develop pathways and workflows for patient care, in partnership with administrators, allied health providers, nursing colleagues and physicians

2)          Plan, prepare and participate in weekly geriatric oncology multidisciplinary rounds

 

3)          Maintenance of GOLDEN website to keep it current

4)          To coordinate annual events such as educational symposium, department team buildings, allied health or nurses’ attachment under geriatric oncology clinic etc.

 

 

Data Management & Quality Improvement

1)          Collate and compiling of data / internal performance measures to monitor effectiveness of the GOLDEN program.

 

2)          Assist in monitoring compliance with regulatory requirements and reviewing internal performance measures to monitor effectiveness of the GOLDEN program.

 

 

3. EDUCATION DUTIES

Education

Increase awareness amongst institution healthcare providers about the importance of recognizing frailty in the geriatric population and to advocate for appropriate treatment for these patients.

 

 

4. RESEARCH DUTIES

Working with clinical research coordinator (CRC) to maintaining ethics currency of ongoing geriatric oncology studies. (ie. IRB) and data collection for database

 

Qualification Required and Area of Discipline

Bachelor’s Degree in one of the following disciplines:

•            Nursing (preferably with oncology or geriatric experience)

•            Life Sciences (preferably with oncology or geriatric experience)

•            Allied Health (preferably with oncology or geriatric experience)

o            Medical Social Work

o            Occupational Therapy

o            Physiotherapy

o            Pharmacy

o            Dietetics

Strong candidates should have experience in one of the following:

•            Oncology Nursing

•            Geriatric Nursing

•            Geriatric Medicine

•            Medical Oncology

•            Radiation Oncology

•            Health Services / Care Coordination

•            Population Health

•            Cancer Survivorship Care

•            Clinical Case Management
