The Case Management Officer is responsible for assessing, planning, coordinating, monitoring and tracking of cases to ensure continuity of care being delivered to patients. He/She will need to collaborate with a multi-disciplinary team of doctors, psychologists, nurses, medical social workers, and other allied health workers in providing timely and appropriate treatment for the patients.  The Case Management Officer will work with a Case Management Officer lead to provide mental health support to patients.

The ideal candidate should have a passion for serving in the public sector to meet the mental healthcare needs of Singapore residents. He/she should be highly motivated to learn, grow personally and work independently. The Case Management Officer works under the supervision of the Case Management Officer lead and the Programme Director.

Duties and Responsibilities

  • Screen and triage cases for case management.
  • Administer and collate clinical questionnaires and assessments before patient’s visit to psychiatry clinics.
  • Work with the identified patient to plan and coordinate his/her care, activities and discharge based on holistic assessment of his/her clinical, physical, psychosocial, and other identified needs.
  • Provide psychoeducation and supportive work for patients and/or their families/ caregivers.
  • Ensure patient’s treatments and consultations take place timely. Assist with default management and rebooking of appointments.
  • Function as a resource person for services/service linkage with community partners and social service agencies. Keep up to date with evolving community resources/ services and support community partnership engagement.
  • Collaborate proactively with hospital’s multi-disciplinary team, provide coordination and continuity of patient care across the healthcare continuum with primary care and community mental health partners to facilitate smooth transition of patient to the home and into the community.
  • Plan, coordinate and conduct educational activities for community partners and the public.
  • Monitor and maintain programme database and indicators.
  • Create and promote patient education materials.
  • Participate in projects related to case management and clinical pathways.
  • Perform any other duties and responsibilities as required by the department.

Requirements

  • Recognized/Relevant Degree in Psychology, Social Work, Counselling, Nursing (with clinical training in mental health) or related field.
  • Minimum 2 years of experience in managing mental health conditions.
  • Prior work experience with managing mental health conditions in a hospital's inpatient or outpatient setting will be an advantage.
  • Good computer literacy skills.
  • Strong organizational, interpersonal and communication skills.
  • Ability to communicate in dialects will be an added advantage.
  • Able to mulit-task, work independently and collaborae as a team.
