Job Purpose

NUH IPMDA (Integrated Program for Management of Depression in Adolescents) aims to provide holistic and integrated care for adolescents with depression. IPMDA Case Management Officers are responsible for assessing, planning, coordinating, monitoring and tracking of cases to ensure continuity of care being delivered to our patients. He/ She will need to collaborate with a multi-disciplinary team of doctors, psychologists, nurses, medical social workers, and other allied health workers in providing timely and appropriate treatment for the patients.

Duties and Responsibilities

Clinical Roles

  1. Provide emotional support for youths
    1. Conduct intake calls and intake sessions with new patients to introduce the programme and our services.
    2. Provide clinical and psychosocial assessments of patients referred to IPMDA, ascertaining diagnosis and clinical goal planning.
    3. Provide psycho-education and signposting of resources for patients.
    4. Provide supportive counselling sessions and telecommunication support.  
    5. Provide supportive sessions both inpatient and outpatient.
    6. Monitor and encourage medicine and appointment adherence.
    7. Provide relapse prevention advice when needed.
    8. Assess the needs and risks of patients.
  2. Provide emotional support for parents
    1. Psychoeducation for parents regarding patient’s care and treatment.
    2. Provide relevant resources to parents for support
  3. Be the point of contact and liaise between patient, family, external stakeholders and the hospital.
    1. Integrate and coordinate various services or resources (i.e. schools, school counsellor, FSCs, protection services etc.) for patients and parents.
    2. Liaise with relevant stakeholders (i.e. public, school staff, external organisations) regarding patient care.
  4. Collaborate with multidisciplinary team (i.e. Psychiatrist, Psychologist, MSW, Occupational Therapists, Art Therapists)
    1. Collaborate with care team to formulate patient cases and provide optimal care for patients.

Administrative Roles

  1. Administrative
    1. Triage and assess suitability of referral cases under the supervision of a psychiatrist
    2. Data collection/Research
      1. Collect, clean and maintain data in patient database for clinical and research purposes.
      2. Assist in tracking, maintaining and reporting of quality clinical indicators to provide regular statistical report and updates.
    3. Collaborate and maintain partnerships with external organisations and stakeholders.
    4. To be familiar with the community resources, maintain a list of available resources in the Western Region, and keep the list updated.
    5. Initiate and coordinate discharge planning for patients.
    6. Perform other duties and responsibilities, as necessary, in the overall operation of the clinic.
  2. Assessments
    1. Administer brief assessments when necessary (MMSE, Conner’s Continuous Performance Testing).  

 Job Specification/Requirements

  • Recognized degree (preferably in Psychology, Nursing or Healthcare related)
  • Trained and accredited Care Management Officer
  • Has appropriate experience in Mental Health Care
  • Experience working in mental health services in inpatient or outpatient settings
  • Nursing education with Advance/Post Graduate Diploma in Mental Health
  • Computer literacy skills – MS Office (Word / Excel / PowerPoint)
  • Strong organizational, coordination, interpersonal and communication skills
  • Ability to respond to common inquiries or complaints from physicians, staff and patients
  • Ability to multi-task and work independently and work as a team
