- Assist Allied Health Professionals in assessing patients' clinical parameters and standardised patient outcome measures
- Educate patients and caregivers on general nutrition advice within patient and family education materials
- Educate patients on relevant nutrition and self-care knowledge
- Chaperone or provide translations for Allied Health Professionals when needed
- Assist in lifestyle and preventive care programmes within the polyclinic and in the community.
- Assist in preparing patient and family education materials for health promotion and preventive events within the polyclinic and in the community
- Assist in updating departmental resources
- Assist in delivery of group education sessions within the polyclinic and in the community
- Co-lead delivery of group education sessions within the polyclinic and in the community under remote supervision of the clinical dietitian (Dietetic)
- Assist in collecting data for quality improvement projects and or research
- Perform nutrient analysis of recipes
- Conduct nutrition screening and/or diet history under remote supervision of the clinical dietitian
Requirements:
- Min Diploma In Food Science, Nutrition or equivalent
- WSQ Higher Certificate in Food Services will be an advantage
- Preferably with at least 2 or more years of clinical experience in a healthcare setting
- Proficient in MS Excel and Word
- Familiar with video, design software and AI Programmes will be an advantage
- Willing to travel to different polyclinic locations as required
