Overview

Reporting to the Senior Assistant Manager, you will engage patients and caregivers through counselling and outreach, providing information on the NUHS@Home programme and supporting informed decisions about hospital-at-home care.

 

Job Responsibilities

You will be responsible for the following:

  • Verifying patient contact and residential information
  • Explaining programme requirements and care processes
  • Addressing patient and caregiver concerns and facilitating admission into the programme
  • Escalating complex cases or patient concerns to the clinical team where appropriate
  • Collaborating with intake colleagues to ensure a smooth transition for patients into the NUHS@Home service
  • Maintaining accurate and timely records of counselling sessions and outcomes in the relevant platforms
  • Participate in improving work processes and perform other ad-hoc responsibilities when required.

 

Requirements

  • Able to commit to weekday shifts from 8.00am to 2.00pm, with availability on weekends and public holidays from 8.00am to 11.00am on a rotational basis. Working hours are subject to change based on operational and business needs.
  • Driven and dedicated, with a commitment to delivering a smooth and positive admission experience for patients and caregivers
  • Strong communication skills with the ability to explain and convey information clearly to patients and caregivers
  • Empathetic and patient-centred approach when addressing patient and caregiver concerns
  • Ability to manage multiple referrals and follow-ups in a fast-paced environment
  • Ability to work independently and exercise sound judgement when managing patient interactions
  • Ability to work effectively with colleagues across clinical and administrative functions, including doctors, nurses, and other healthcare professionals
  • Comfortable working in a clinical or hospital environment and interacting with patients of varying conditions
  • Proficient in digital tools and able to adapt to various platforms and systems
  • Eager to learn and grow within the role
  • Bilingual ability may be advantageous to communicate with patients of diverse backgrounds
  • Prior experience in healthcare, patient services, or customer service will be advantageous
