Manager, Transitional Care and Community Partnership

(Tengah General and Community Hospital)

 

The Manager will lead the implementation of community and population health initiative for TGCH, translating the department’s strategic direction into operational workplans, programmes and partnerships.

 

Grounded in Asset-Based Community Development (ABCD) principles and community-centred practice, the Manager will operationalise engagement strategies developed by the Head of TCCP, working alongside residents, community groups and partner organisations to activate local assets and build community capacity.

 

The Manager is responsible for day-to-day management of assigned-workstreams, supervision of junior staff and development of operational system and workflows required to support TGCH’s opening. The role requires a practitioner who can translate strategic intent into practical action, manage complexity across health and social care settings and build operational foundation needed for long-term community wellbeing.

 

Job Description:

 

Community Development and Community-Centred Practice

  • Operationalise the department's community engagement strategy, translating ABCD principles into concrete programmes and activities that build local capability and participation. This includes coordinating community engagement across assigned areas, maintains working relationships with key stakeholders, and tracks outcomes to support continuous improvement.

 

Partnership Development and Coordination

  • Develop and manage operational partnerships across the health, social and community sectors, ensuring that day-to-day collaboration is productive and that joint deliverables are met.
  • Support the Head of TCCP in senior-level stakeholder engagements and ensure that issues requiring strategic decisions are escalated promptly.

 

Population Health and Community Hub Operationalisation

  • Lead the operational planning and activation of the Community-Based Hub, working within strategic parameters set by the Head of TCCP. This includes developing governance processes and workflows, managing partner onboarding, and establishing monitoring frameworks to ensure the Hub is operationally ready ahead and supporting the management and reporting of grant funding received for the Hub.

 

Project & Team Management

  • Supervise and develop junior staff, manage team performance, and contribute to departmental planning and governance.
  • Serve as deputy to the Head of TCCP, ensuring operational continuity and representing the department at working-level forums as directed.

 

Job Requirements:

  • Bachelor's degree in Community Development, Public Health, Social Work, or a related field.
  • At least 8 years of relevant experience in community health, care integration or community development, with at least 3 years in a supervisory or team lead capacity.
  • Demonstrated experience in programme implementation and operational management in community health, population health or community development settings.
  • Familiarity with Asset-Based Community Development principles and participatory engagement methods.
  • Competence in programme evaluation, outcome monitoring and basic data analysis.
  • Ability to coordinate across sectors and manage complexity in health and social care settings.
  • Ability to work in a fast paced and dynamic work environment, quick thinker and an effective problem solver. Calm, decisive, and resilient in high-pressure situations.
  • Attention to detail, ability to prioritise workload, be well organised and multi-task in a smart and timely fashion.
  • Possess strong analytical, communication, written and presentation skills.
  • Ability to work independently, in a team and across teams with minimal supervision.
  • Possess exceptional interpersonal and communication skills with the ability to develop strong working relationship at various levels.
  • Proactive attitude and determination to ensure required outcomes are achieved and takes responsibility.
