The Job Holder is responsible for managing the implementation and evaluation of strategies and programmes to enhance the delivery of community care and improve population health in the western region under NUHS Regional Health System Office. This is to be achieved in close collaboration with key internal and external stakeholders and partners.
Job Responsibilities
1) Support the development and implementation of plans to improve population health under Health District @ Queenstown
- Support and coordinate RHSO overall strategic planning efforts to improve population health for the Queenstown population, through development and implementation of targeted outreach and engagement strategies to residents
- Assist with the operational planning, development and implementation of workflows and processes to ensure smooth operations at the physical centre
2) Support in project planning and management
- Manage projects by developing project plans, budget, resource planning, timelines and deliverables
- Support development of initiatives by providing timely updates to project leads and key stakeholders, working closely with operational and clinical teams, and ensure alignment with project objectives
- Track and evaluate the effectiveness of these strategies, leading to data-driven improvements in strategy and operations
3) Support in stakeholder engagement and build partnerships
- Provide secretariat and administrative support for stakeholder partnerships through organising and facilitating meetings e.g. NUHS institutions, government agencies, social service agencies and other community stakeholders
4) Others
- Successful execution of any other duties assigned by HOD.
Job Requirements
- Bachelor degree in any discipline
- At least 2 years of relevant working experience
- Knowledge and experience in project management will be advantageous
- Experience in the Singapore healthcare system and/or knowledge of integrated care models will be advantageous
- Strong analytical abilities
- Project management skills
- Strong written and verbal communication abilities
- Adept in people skills to foster buy-in and collaboration
