Senior Medical Social Worker

(Tengah General and Community Hospital)

 

About the Role

As a Senior Medical Social Worker, you will play a key role in the development and delivery of medical social work services for Tengah General and Community Hospital. In addition to supporting patients and families with complex social, emotional and practical needs, you will collaborate with multidisciplinary stakeholders in shaping workflows, programmes and service models that support integrated, person-centred care.

 

Job Description:

  • Contribute to planning of medical social work services for Tengah General and Community Hospital, including space, design, workflow and manpower considerations.
  • Collaborate with clinical, allied health, operational and administrative stakeholders in planning of medical social work services.
  • Provide end-to-end case management support and appropriate level of care through assessment, care planning, interventions and evaluation of care, including complex, specialised and high risk cases.
  • Coordinate patient care with multidisciplinary teams
  • Lead or assist in planning and implementation of existing and/or new programmes which enhance psychosocial well-being of the community.
  • Network and collaborate with community partners.
  • Comply with clinical and service standards.
  • Support administrative duties and responsibilities as assigned.

 

Job Requirements:

  • Bachelor’s degree in Social Work, Graduate Diploma in Social Work or Masters in Social Work from an accredited tertiary institution.
  • Preferably with a minimum four years’ experience as a social worker.
  • Accreditation as a registered social worker with the Social Work Accreditation and Advisory Board (SWAAB).
  • Adhere to the following ethical principles: autonomy (ensuring patients' confidentiality, self-determination, privacy and right to truth), beneficence, non-maleficence and justice.
  • Ability to multi-task and work under tight timelines.
  • Good analytical and organisation skills.
  • Excellent communication, problem solving, presentation, negotiation and interpersonal skills.
  • Proficiency in additional languages or dialects to engage patients from diverse backgrounds is preferred.
  • Familiarity with digital tools and AI-enabled solutions will be an advantage.
