Senior / Staff Nurse (NUH Dialysis Centre)

Specific Roles and Responsibilities

1. Perform assessment, identify areas of needs and manage patient with different vascular access undergoing HD/HDF.

2. Perform HD / HDF/ PD according to the treatment plans and manage safety and complications accordingly.

3. Assess and review patient’s blood results and clinical parameters, collaborate with physicians to adjust dialysis regimen and medications as prescribed.

4. Assess and perform quality assurance of dialysis water and dialysis fluid in accordance with hospital policy and MOH Guidelines. 

5. Provide information and education to individual HD/HDF patients on the care of vascular access, dietary and fluid restrictions and offer patients counselling when required. 

6. Perform and conduct nursing assessment of patients’ suitability for PD. 

7. Provide PD training and assessment of competency to individual patients and caregivers to enable them to perform PD independently at home.
